• Applicable Certificate of Registration or license which may be issued either by the Bangko Sentral ng Pilipinas ("BSP"), Securities and Exchange Commission ("SEC"), or the Cooperative Development Authority ("CDA");
• Articles of Incorporation from SEC or Articles of Cooperation from CDA, depending on the entity.
• Secretary's Certificate naming the authorized representative. (See the sample template here)
• Any additional and relevant documents as may be required by the CIC, if applicable.
